What kind of counselors do you have?
All counselors are
professionally trained and certified in their therapy discipline.
LCS' staff is accountable to Florida licensure requirements.

What are your hours?
All clinical sessions are by appointment. The office is open 8:30 - 5:00 (Mon-Thurs) and 8:30 - 4:30 (Friday). Some clinical appointments are available in the evening.

Do you take
Insurance?
Yes - each therapist has their own set of panels. Please check by THERAPIST NAME AND ADDRESS with your insurance company's mental health provider. If it is a managed care program, you will need to get the authorization number, the number of sessions authorized and the amount of your co-pay. This info should be received by our office prior to your first appointment.

How much does a session cost?
There is a standard professional fee ($135) for a therapy session. (There is a $15 one time fee to set up the chart and complete initial evaluation.) If there is no insurance, the amount paid will be dependent on gross family income and family
size.

Do you take Credit Cards?
Yes -- Master Card or Visa, check or cash
Payment is due at time of visit.

How long are the sessions?
Approximately 45-50 minutes.

How often will my sessions be?
Scheduling arrangements will be made with your therapist dependent on nature of problem and availability of both parties.

Do you have to be Lutheran (or Christian) to obtain services?
No - ANYONE regardless of religious belief is welcome. Our mission is to provide quality professional services to the entire community.

Do you offer free depression screening?
Yes, Dr. Mary Jacob will see you for a 15-20 minute session after you complete a brief questionnaire in this office. At that time you may be referred for a complete evaluation if it is indicated.

Can I
complete my intake paperwork before my appointment?
You may print out the following 4 pages, complete them and bring
them with you to your first appointment. They must be
completed before you meet with your counselor. (Right
click on each form individually and "Print Picture" for each page.)
